Dormitory Event Registration Form

Two dormitory residents must take responsibility to manage any dorm event; please provide these resident names and contact information below.

Submission of this form indicates that the residents responsible for the event:

1. have secured approval from all dormitory residents to hold the event,
2. will ensure the gathering ends by or before 11 p.m., and
3. will be responsible for clean-up at the end of the event.

Please submit at least eight hours before event.

Events must end by 11 p.m.
